Hetal Baman (00:07.645)
hello my friend
Charis Hamilton (00:10.000)
yeah
Hetal Baman (00:12.625)
i am so excited to have you on the podcast for everyone who doesn't know chris is one of my very very close friends one of my best friends and we've known each other for how many years man like since was it since high school or middle school probably middle school right
Charis Hamilton (00:36.560)
yeah middle school yeah good times
Hetal Baman (00:39.885)
oh my gosh i don't even remember the first time we met like what
Charis Hamilton (00:43.400)
i don't either my gosh i probably like followed you around and yeah
Hetal Baman (00:50.265)
i doubt it i was so shy back then in middle school um that's just so funny oh my goodness so i've just i am honored to have you on the podcast i think it's this topic is something that i didn't know about just like a year ago and you currently practice physical therapy
Charis Hamilton (01:10.580)
hm
Hetal Baman (01:18.085)
and i remember you telling me that you wanted to start a practice within pelvic floor therapy and knowing the person that you are you've already set up the whole your whole office your whole thing and i was like how did i not know about any of this
Charis Hamilton (01:19.700)
m hm
Charis Hamilton (01:41.360)
i'm more like if i want to do it i'm just going to do it you know i better just go full force and full speed ahead
Hetal Baman (01:49.665)
i know you're very very quiet until like you got everything set up yeah that's just that's just you in a nutshell i think it's so great so your physical therapist you started a practice in public floor therapy tell me why why pelvic floor therapy what was the first thing that what was the first moment when you were like you know what this is something i'm really passionate about
Charis Hamilton (01:51.780)
is de yeah
Charis Hamilton (01:57.820)
yeah
Charis Hamilton (02:15.000)
m hm
Hetal Baman (02:20.445)
i want to serve other women who are going through these these problems these issues
Charis Hamilton (02:25.720)
yeah so you know i'll kind of give you a background first of schooling you know we go to school for years and years to get your first you have to get undergrad and then you get you can't get your doctor at which i believe is what they all recommend now for physical therapy and when i went back to school back in the day i guess they honestly didn't even really have a full course on women's health and it was maybe like
Hetal Baman (02:43.185)
hm
Hetal Baman (02:46.405)
uh uh
Hetal Baman (02:54.005)
right
Charis Hamilton (02:55.420)
one class or for three hours and that was it and then you know you can go the beauty of our physical theapties you can work in hospital out patient rehab centers long term care and those are all great settings to work in but when i had weston my first daughter things just changed in your public flora area now you start feeling extremely weak you can possibly have some leaking some symptoms of heaviness and
Hetal Baman (03:19.105)
m hm
Hetal Baman (03:24.005)
a
Charis Hamilton (03:25.400)
the type of person when i don't know things i kind of get scared and i'm like oh boy i feel like i should know this you know and yeah yeah right right and i was like i can't believe like this is happening i exercised throughout school throughout after graduate school during my week
Hetal Baman (03:31.065)
m
Hetal Baman (03:34.145)
especially being a physical therapist like and having all of that schooling
Hetal Baman (03:46.905)
okay let me let me just say this woman did cross fit like i tried cross fit for like a month and i was like okay this is little lot
Charis Hamilton (03:50.760)
yeah
Charis Hamilton (03:58.280)
a lot and so when i started feeling extremely weak and had symptoms of heaviness and prolapse it's like how how dare you
Charis Hamilton (04:11.160)
so you know after i had weston had all these symptoms that's when i kind of started going back to continue education and just learning so much more about pelvic health and then you know i'm returning to cross fine and of course the first of squad i was like who this is not the same and you know remember going back six weeks to i already had my clarence from o b she says go back to whatever you were doing which was cross finn remember the first day i went back i was like this is not right i should know
Hetal Baman (04:30.665)
m
Charis Hamilton (04:41.040)
be here way to week but why did nobody tell me this why did nobody tell me that the six week mark is really not a good indicator of when you should go back to the level of activity you were doing before and so you know as i'm studying as i'm like kind of diving deep diving into pelvic health i realize just like so many things and you know and then going back to cross it and i'm realizing wow like a lot of women
probably do not know this you know i see a lot of women trying to exercise and cross it which is great but you know you should still be watching your form you should still be watching your breath if you're holding your breath how your public for muscles are they engaged or not engaged during these activities and then especially post partem you know you really everyone should really be seeing a public for therapist kind of give you that clarence of okay hey are you ready to
back to running are you ready to go back to plates are you ready to go back to even walking uphill because so many things can happen if you just try to rush back right away especially if you've had a very intense you know pregnancy labor or delivery do we have any tearing was there an emergency c section things like that so
Hetal Baman (05:48.125)
right
Hetal Baman (06:04.425)
all of this is so important i just want to tell our listeners that you might hear a little bit of gurgling or a little babbling in the background because we have a little baby little jase cherrises third daughter so it's mom life you just got you know how it is just got a roll with it okay so i want to take things really really basic so what
Charis Hamilton (06:05.100)
m
Charis Hamilton (06:09.000)
oh
Charis Hamilton (06:12.880)
we have a seal
Charis Hamilton (06:20.300)
yeah
Charis Hamilton (06:25.800)
got roll with it
Hetal Baman (06:34.805)
what is the pelvic floor
Charis Hamilton (06:36.380)
so you're pelvic floor you know it is pelvic there are pelvic for muscles so pelvicflor muscles are the muscles that kind of live in your pelvic bowl which is your between your pelvic bones from your your big bone all the way to your tail bone and these are the muscles that are responsible for stability support they have spencteric roles which means they are the ones that control when you release gas when you release your and things that come in and out of that
vaginal canal and all the holes that you have in your anatomy because it shares that detachment with your pelvis and your hip your tail ball and your sacrum i consider your pelvic for really to be a combination of your front your core your lower abdomens um your back muscles your glue muscles your inner size all of that kind of i believe is considered your pelvic floor but specifically you have your pelbicflor muscles
Hetal Baman (07:36.405)
okay and how does that anatomy change when you start to have babies
Charis Hamilton (07:43.020)
well during pregnancy you know obviously you're going to gain a lot of weight you're going to gain a lot of weight there's going to be a lot of pressure just from being pregnant so a lot of times you know if you are not aware of your posture posture changes throughout pregnancy people just because the weight is forward then you know sometimes i see a lot of women their pelvis moved forward and then they're starting to kind of tilt and then when you start tilting your low back starts to
Hetal Baman (08:00.205)
m hm
Charis Hamilton (08:12.920)
an increased arch and then because depending on if you're carrying twins or the way you're walking then we can start to literally see a waddle that is rotating um in the wrong direction and then you know a lot of things can be compensated if you're not aware and then you go through labor delivery throughout that time all your there's a lot of hormones that come in to play a lot of ligaments are kind of more relaxed just to come
Hetal Baman (08:24.165)
m
Charis Hamilton (08:43.220)
help you with pregnancy um so there are a lot of changes throughout pregnancy and then you have your labor and delivery a lot of things can happen during labor and delivery you know where
Hetal Baman (08:54.045)
right
Charis Hamilton (08:59.960)
pelmicflor muscles can be extremely lengthened or if you've had a traumatic labor and delary they can kind of get into a tightened position
Hetal Baman (09:11.165)
so who who do you see the patients that come to you what do they most complain about
Charis Hamilton (09:20.480)
yeah so i see a whole bunch of patients whether you're pregnant or not pregnant if you're a woman i can see you and a lot of times i see there's a lot of leaking um and there's different types of leaking are you leaking only when you're coughing are you leaking only when you're sneezing are you just leaking
Hetal Baman (09:39.405)
and when when you mean when you say leaking what do you mean by that
Charis Hamilton (09:41.780)
leaking as you can leak urine or you can leak faces so urn or bowl in continents and that's where your public for muscles can play a role because one of their roles is to be able to control the release of our the release of gas and stool um see a lot complaints of heaviness they feel something it can be falling out again or it's that sensation it might not be literally
Hetal Baman (09:47.865)
m
Charis Hamilton (10:11.620)
happening but it's that sensation of wow it feels like something s falling out it feels heavy i eel like there's something bulging out or i actually see a lot of low back pain a lot of hit pain because of the changes that could have happened during pregnancy
Hetal Baman (10:20.865)
m
Hetal Baman (10:29.365)
and so when you say bulging out what is that
Charis Hamilton (10:31.900)
it's a lot of people complain about the pressure of things coming out like oh my gosh i had my baby but i feel like something is still hanging out something feels like it's bulging out like their internal organs you know like
Hetal Baman (10:46.265)
m
Charis Hamilton (10:46.740)
um and that should not feel that way it's very common to have those complaints but that's not normal
Hetal Baman (10:57.545)
gotcha i want to say that in doing some research i saw that in developed country is like the u s one in every four women actually experienced this at least one or more pelvic floor disfunction um issues which is like we're talking about twenty five or twenty five under trying to do at twenty five percent of women
Charis Hamilton (11:16.700)
right
Hetal Baman (11:28.385)
and what do you think the percentage of women who actually know about pelvic floridas function is
Charis Hamilton (11:34.840)
right probably just twenty five per cent twenty five maybe fifty percent i mean now with the help of social media i think it has definitely increased and now you know i'm pretty open so i have no problems talking about p and poop and but i think the more it's talked about the more the conversation can be you know easier but
Hetal Baman (11:54.405)
uh uh
Hetal Baman (11:57.805)
m m
Charis Hamilton (12:04.700)
so just more you know if more people are open about talking about it and realizing that all these ping and pooping problems are not normal then maybe more women will seek out more help
Hetal Baman (12:20.645)
i just wanted to talk about
Hetal Baman (12:24.445)
you had mentioned that you went to your b g n and she was like okay you're ready you can you can go back to doing cross fit you're all good where do you think the disconnect is there because you would think that maybe your b would say oh you need to go to therapy or you need to do this or that where do you think the disconnect is
Charis Hamilton (12:26.940)
hm
Charis Hamilton (12:44.380)
right
Charis Hamilton (12:48.280)
you know i feel like there's so many factors obs they're so busy or at least the one i went to was just so busy and unfortunately they just don't spend more than more than fifteen minutes with you in the office so the quick the quick easy thing to say if you did have a easy pregnancy and no complications and pretty easy labor and delivery without any complication they're just like okay just go back but a lot of
Hetal Baman (13:04.645)
right
Hetal Baman (13:17.785)
right
Charis Hamilton (13:18.080)
be you know if they took the time to explain like hey you know you would benefit from public or therapy you know that would be great but i think a lot of times they just don't have that time to explain it to patients and a lot of ops are also surgeons there focused on very you know very medical routes through medicine through surgery and
therapy is more of a conservative treatment you know we're not using medicine we're using just muscle in to work on your strength and range of motion by a lot of ops you know and there's more holistic ones now who are more into natural and more conservative treatment but a lot of ovis are just like okay we can schedule and induction okay let's schedule your c section well you know so you know you just find the practitioner
Hetal Baman (14:13.825)
right
Charis Hamilton (14:18.140)
it vibes with you and shares the same you know the feelings and that can help you know with your pregnancy and post pardon
Hetal Baman (14:26.905)
hm
Hetal Baman (14:32.405)
gotcha so when people think okay i have some type of pelvic floridas function
Hetal Baman (14:43.285)
what do they first think of like that what do they think they need to do because i saw on your on your instagram there's a post that says you mean it's not all just keggales
Charis Hamilton (14:56.840)
yeah yeah and so yeah a lot of people if they had that feeling out like oh i feel like something is falling out a lot of people's automatic responses i better pull that back in i better try to suck it up and tighten it up so that it doesn't feel like it's falling out but honestly giggles is really not functional and a lot of people actually don't need it you know back in the day they said do
Hetal Baman (15:15.305)
right
Charis Hamilton (15:26.600)
a hundred keigles a day you you'll be sure to have a strong public floor muscle but i don't see with more research and more functional assessments that's really not the case you know so a lot of people that come in they're like well i tried doing keegles but now i feel worse or my pain has gotten worse why is that you know and a lot of times i find that women have an over active public for muscle
um where that's not functional and that's not strong either and then we have to kind of undo that
Hetal Baman (16:03.645)
huh so when you mean when you say over active pelvic floor muscle like what does that really mean
Charis Hamilton (16:06.560)
m
Charis Hamilton (16:08.460)
so because it's a muscle every muscle should kind of just like my biceps should be able to lengthen here is over lenken which when you're at an over lenkedend position you're also not weak and also not functional no one should be carrying a gallon of milk with a hyper mobile elbow but your muscle should also be able to contract relax contract have a controlled relaxation if have any over active muscle
Hetal Baman (16:15.005)
right
Hetal Baman (16:23.885)
okay
Hetal Baman (16:29.285)
gotcha
Hetal Baman (16:36.905)
uh huh
Charis Hamilton (16:38.280)
you're kind of only working in this range of motion
Hetal Baman (16:42.945)
like the contractions okay
Charis Hamilton (16:43.840)
right right and so if you say okay let's do a hundred kegs or a hundred push ups or even a hundred squats a day that muscle usually tends to get over active it tends to either just spasm because it's just working too hard and work
Hetal Baman (17:02.245)
it's like the concept of resting in between like the amount of rest versus am i right or am i
Charis Hamilton (17:09.460)
yeah like a lot of people don't work leg days back to back unless you're doing you know because a lot of time you can overwork those muscles and when a muscle is overworked so if you're muscling if you're trying to work your legs again but they're already sore you're definitely not going to get the same results as you did if that muscle was rested and not sore
Hetal Baman (17:14.585)
right
Hetal Baman (17:40.005)
that makes no that makes so much sense
Charis Hamilton (17:42.020)
yeah so when people are like oh i did a hundred giggles for thirty days you're thinking oh my goodness wow that is a lot of kegs your pelvic muscles are really delicate i like to say you know they are not big muscles there you know they're very powerful there's you know they play a big role but you do not need to know muscle you should do every day for a hundred times you know so it's unfortunate that back in the day they said do a hundred
goals for so much and now people come with me complaining of okay there's pain i thought i was supposed to feel better with this heaviness but i feel worse or why am i leaking more you know so there is adverse reactions when exercise is not prescribed the right way or the type of exercise is not prescribed the right way
Hetal Baman (18:37.985)
right
Charis Hamilton (18:39.900)
is being blown right now
Hetal Baman (18:40.865)
and this is i mean it's it's so interesting because i haven't you know obviously i don't have children at this point but and maybe if i did then maybe i would know about it but when you think about our older generations like nobody's really known about ellice floor therapy like and like the research that you were saying was h do a hundred kiggles every day and
Charis Hamilton (19:00.620)
right
Hetal Baman (19:11.045)
then you'll be fine and i think it's just recently coming to light that that's not the only thing you should be doing or that's not even what you should be doing in the first place um and so just it's just very interesting that this was this has been coming up just like in the past decade or so you know
Charis Hamilton (19:22.500)
right right
Charis Hamilton (19:38.280)
hm
Hetal Baman (19:42.225)
that's that's why i i'm so interested about it because you know we both have immigrant parents um my parents are from india your parents are from china and
Charis Hamilton (19:48.580)
hm
Hetal Baman (19:56.345)
did your parents even know like when you said i'm going to become a pelvic floor therapist like what what did they think about that
Charis Hamilton (20:03.780)
well i don't i unfortunately don't think they really know what physical therapy is but that's just because we have a language barrier but if i were to explain it to them um i don't think they would know because
Charis Hamilton (20:19.960)
i really you know especially in my culture in my household we never talked about ping pooping intercourse things like that so even opening up that community like that conversion will just be like way but that's that makes it very limiting then
Hetal Baman (20:28.325)
yeah
Hetal Baman (20:33.325)
that yeah you'd have to i know that's
Hetal Baman (20:42.125)
it does i think it's also the cultural barrier so like when you when you for example like in india it's the same thing you know i think it's like this asian culture where you keep all of that all of the sex talk and the and them anything that's related to your productive organs like even when you get your period like that stuff is like under wraps like nobody knows about any of this
Charis Hamilton (20:43.960)
yeah
Charis Hamilton (20:57.300)
m hm
Charis Hamilton (21:09.680)
right
Hetal Baman (21:11.965)
for like your sister or like or your mom you know i mean minus the sex talk but like it's a huge cultural thing and so that's what i did some digging and i found a couple of studies um that focused on pelvic floor therapy within the country of benladesh a couple of studies by this one guy named r m islam and he had found that
Charis Hamilton (21:15.400)
m hm
Charis Hamilton (21:22.820)
right
Charis Hamilton (21:34.700)
hm
Hetal Baman (21:43.825)
thirty five point three so basically a more than a third of women in bangladash had more one or more public floor dis functions like say he had mentioned that urinaryincontinents was the most common dis function and like the odds of urinary in continents were significantly greater with increasing poverty i mean it makes sense right because you don't
Charis Hamilton (21:53.600)
m hm
Charis Hamilton (22:12.660)
m
Hetal Baman (22:13.705)
have who knows if these people are even going to the doctor you know
Charis Hamilton (22:15.940)
is it right right
Hetal Baman (22:21.125)
it's just so it was just interesting to me like more than one third and then one of every six women who had pelvic floridas function had pelvic organ prolapse
Charis Hamilton (22:34.600)
m hm
Hetal Baman (22:38.585)
and i think like what you said that the whole cultural barrier the language barry and all of that stuff even within the country like low and middle income country like
Hetal Baman (22:50.965)
i'm just too embarrassed to talk about my issues down there and i don't know what to do so i'm just going to continue on with living like this
Charis Hamilton (22:53.100)
m hm
Charis Hamilton (23:04.640)
right right
Hetal Baman (23:07.605)
and have you seen things like that here as well with women being super embarrassed about even going out or wearing tight clothing or things of that nature
Charis Hamilton (23:20.280)
um i've seen a lot of women where they're they're like they come to me they're like i've tried this this and this and five years later like i still have these symptoms you know and it's it's and then they're like okay they always say like i don't know if this is t m i but i'm like nothing here is t m i you know and but it's funny when everyone says that because they're like everyone must feel like
Hetal Baman (23:45.025)
yeah
Charis Hamilton (23:50.080)
it's very embarrassing to talk about or it's too much it literally is too much information but not to a public flortherapist you know and then i find a lot of times that you know like wait these sometimes started five years ago and now you're coming to see me now so it's like maybe because they didn't know about it or they just brushed it off or they thought things would naturally get better by themselves um yeah so i can definitely i'm sure that happen
Hetal Baman (23:53.425)
right
Charis Hamilton (24:21.200)
elsewhere too
Hetal Baman (24:23.905)
yeah that's like it's like the whole i'm embarrassed to talk about this and your job is your job is actually to talk about this so it's like
Charis Hamilton (24:35.220)
xactly and then so when people wait it's not t m i'm like no they're like relieved in a way because it's like while someone is finally listening to something i've been embarrassed about you know which is it's it's very nice to i love my job you know because you see like that mind blowing like they're just like oh my yeah yeah there like wait and the good thing is you know
Hetal Baman (24:58.405)
the mind blood in moi
Charis Hamilton (25:06.320)
your public cormac because it's kind of like in the center of your core and it's so part of just all your systems that you fix you address your public fore muscles and a lot of other things kind of follow along you know so there's improvement you know you can have symptoms of prolapse that heaviness and then next you know you're not leaking as much and the next you know like that hit pain is going away so it is it isn't really
nice specialty to treat
Hetal Baman (25:40.185)
it's like it's almost like a therapy session
Charis Hamilton (25:43.940)
and but i love being able to provide that for women because i think a lot of times you know in the health care you go in your doctor's office you have five to ten minutes to tell them all your problems i'm going to give you more time and i think a lot of women need that you know bea realized at the end of session like wow all of this kind of is all related or it all makes sense or all comes together
Hetal Baman (26:02.525)
yeah
Hetal Baman (26:11.425)
well would you say do you ever work with um your patient spouses or significant others and talk to them about how they can support their you know their person
Charis Hamilton (26:26.080)
yeah yeah i definitely you know they're always welcome to bring their spouse their boy friends their babies and you know if everyone wants to have this conversation yes so it's you know but i want of women you know kind of want to dress it on their own um it all depends on the person i guess
Hetal Baman (26:50.425)
yeah
Charis Hamilton (26:50.680)
but yeah i'm definitely i've seen spouses in there and it's nice when they realize there as so many factors that can be going into um what they're feeling and then i kind of go over the process of pregnancy labor delivery and then they have like a real truer appreciation of what women can do you know they're like
Hetal Baman (27:15.785)
it's like i can i can fix this i don't have to live with this forever
Charis Hamilton (27:19.560)
yeah yeah
Hetal Baman (27:23.325)
like i said mind blown mog
Charis Hamilton (27:24.440)
yeah yeah and that's
Charis Hamilton (27:28.260)
that is nice to have women realize all of that you know
Hetal Baman (27:34.885)
is so important so if there was one book that you would recommend um women if they want to just learn about this maybe if they're starting to feel symptoms and they just want to learn a little bit what would you recommend them read
Charis Hamilton (27:48.860)
m
Charis Hamilton (27:50.940)
um i think i gave you the titles by like i really like restoring your pelvic floor and healing pelvic pain even if you don't have any pain right now that book really does go back to the basics and kind of does a little explanation of kind of what i said um which it's always helpful to explain to women that before you go straight into treatment you know so you kind of
Hetal Baman (28:20.205)
right
Charis Hamilton (28:20.900)
get that understanding of what we're working with first so yeah healing pell plane and restoring your public floor those are definitely good books you can just get off online
Hetal Baman (28:32.265)
amazon yeah so i'll throw those links down in the description along with the other three books that she had recommended um so
Hetal Baman (28:46.425)
i think the last question i want to ask you is you know if people have questions for you and where where are you right now so if anybody is listening in your area and is having these symptoms they can come and contact you they can talk to you about all the things that they're going through
Charis Hamilton (29:10.700)
m hm
Charis Hamilton (29:13.660)
yeah i mean you can link my instagram to my business account the maternal fit public physical therapy can message me there i have my email and my phone number and you know if anyone is even interested in public floratherapy i always recommend i can do phone consults a lot of the times but i do prefer you know the initial assessment to be hands on you know so
Hetal Baman (29:42.825)
hm
Charis Hamilton (29:43.440)
now google is great to just google public floor therapist near me and i would recommend calling them and you know hopefully they're able to provide a free complimentary like fifteen minute consult to see if this sounds like the right thing for you if that if that therapist sounds like you know she's going to be listening and helping you with your issues um and then there are directories to have that you can find
Hetal Baman (29:48.645)
right
Charis Hamilton (30:13.400)
